By signing the Client Informed Consent to Share and Release of Information form, the client understands that any information the client shares with an agency participating in HMIS is kept confidential and that only those authorized to input data into HMIS can view the client’s personally identifying information. Release is good for one (1) year and if client received services from another agency and agrees to share their information, they will not have to sign another form within that period. If the client decides at a later date to no longer share client information in HMIS, future information can be locked down.

 

The HMIS Project Consumer Notice explains to the client the Partner Agency participates in the HMIS Project. This notice must be placed in a visible area where clients will have the opportunity to view it.

The NDHMIS Policies & Procedures govern the use of the North Dakota Homeless Management Information System including the roles and responsibilities of stakeholders involved in NDHMIS.

 

The NDHMIS Data Quality Management Plan lays forth the minimum requirements related to data quality for NDHMIS-participating organizations.

 

The Privacy Plan lays forth the minimum requirements related to the joint responsibilities for ensuring that HMIS data processing capabilities comply with HMIS privacy, security and confidentiality policy and procedures for all NDHMIS-participating organizations.

 

This Policy describes standards for the security of personal information collected and stored in the North Dakota Homeless Management Information System (HMIS), as well as personal information collected for the purpose of the North Dakota Continuum of Care (ND CoC) Coordinated Access, Referral, Entry, and Stabilization System (CARES). The standards seek to ensure the security of personal information. This Security Policy (Policy) is based on principles of fair information practices recognized by the information security and technology communities.
