SC Lowcountry HMIS Training Request Form
This form should be completed by the supervisor of the person who needs training. Once this form is submitted, an ICA representative will review and process the request. If you have any questions, please contact the help desk at hmis@one80place.org.
Authorized representatives are strongly encouraged to review the Lowcountry CoC’s training procedure to have a clear understanding of the training process and expectations of users and authorized representatives prior to completing and submitting this form.
